Requirements
- Completed Application Form (Please contact the consulate for the forms)
- Copy of your photo ID
- Police Report
- Copy of Computer Generated Birth Certificate/Copy of missing passport bio page if available
- Completed statutory and Notarized Declaration form (Please contact the consulate for the forms)
- Four (4) most recent Passport size photographs with white background
- Money order payable to the Embassy of Ghana

Ghanaian Parents acquiring passports for their Children below 18 years of age
Requirements
